Shuttle Reviews: Integrated Hazards Review
Boeing reviewed all the waivers and deviations from previously accepted hardware that didn' t meet its specification, although when tested, they were within its factor of safety and there was an acceptable rationale to go forward and fly. Boeing made sure that the rationale was still acceptable and that if there are multiple occurrences of hardware on the vehicle or linking hardware, that there' s not an additive affect that unintentionally drops below the accepted factor of safety.
On the integrated hazard review, Boeing also conducted a system level assessment and documented the hazardous things that can occur during a flight. About 40 new system-level hazard reports will be closed before the shuttle flies again.
