IA/Work-Fit
Data Processing Activity

The Boeing Company and its group of companies seek to maintain the privacy, accuracy, and
confidentiality of personal data that we collect and use.
We have established privacy and security measures
both internally and (where applicable) in our relations with third parties to safeguard
personal information in our data processing activities.
Industrial Athlete Program Privacy Notice Collection and Use of Personal Information
Program Description
Industrial Athlete (IA) is an injury prevention program focused on Boeing manufacturing employees. These voluntary services take place on-the-clock and employees must have supervisor approval to attend. There are three main aspects of the program:
1) Proactive Services: Education and one-time preventive services that focus on program awareness and self-care skills
2) Job Conditioning: a 7 week musculoskeletal training and body mechanics education program, 2X/week for 45 minutes
3) Symptom Intervention: first-aid treatment for musculoskeletal discomfort, to relieve symptoms and identify the root cause and make appropriate positioning/movement adjustment.
Policy Scope
The scope of this policy pertains to the information that Boeing collects and retains on employees to document their participation in the Industrial Athlete program. This includes the initial forms that participants complete as part of Job Conditioning (PAR-Q and Medical Release), the sign-in sheet used for Symptom Intervention (SI Log), and the sign-in sheet used for open gym use (Open Gym Log).
Boeing uses a supplier named Work-Fit to provide program services and to enter program attendance and case notes into their database. Boeing receives a nightly file from Work-Fit that documents daily program usage for each program. This file is used to manage the program, make resource decisions, and evaluate overall effectiveness. The file is stored on an internal secure server and access is restricted to Boeing Industrial Athlete team members only.

� BEMS ID
� Location
� Medical history (Job Conditioning screening)
� Medical Release Forms (from your doctor)
� Dates of program involvement (visits, completions, etc.)
� The reason you visited a trainer or therapist (affected body part, pain rating, and treatment provided)
How Your Information Is Used
The IA program may use your data in a variety of ways, depending on which services you interact with, including to:
� Determine program effectiveness
� Evaluate program usage trends
� Provide aggregated, anonymous metrics for internal Boeing partners (i.e. leadership teams, EHS, etc.)
� Respond to inquiries about employee attendance
� Improve user experience and the quality of our products and services
In the case of internal injury correlation studies involving IA data, your data would be used only if the information can be aggregated and made anonymous, so your name could not be associated with specific health information data. Any analysis using sensitive or highly sensitive data would be undertaken only after Boeing Legal and Boeing Global Privacy have approved.
Authorization to Process Your Information
When you consent to participate in an Industrial Athlete program, you are giving your permission for us (and our supplier) to store your program data to carry our administrative processes as described above.
Protecting Your Information
All data collected is maintained in a secure environment until it is destroyed per established retention schedules. We have implemented and will maintain appropriate technical and organizational measures to protect your information against accidental loss, destruction or alteration, unauthorized disclosure or access, or unlawful destruction. The security features of these databases and processes for using the data have been closely evaluated and approved by Boeing Information Security. The supplier is obligated under our contract with them to follow these same Boeing information protection practices.
Disclosure of Personal Information
Your non-sensitive information (e.g. Name, BEMSID) could be shared on a need-to-know basis (e.g. to confirm program attendance with your immediate manager) or to match with other data (e.g. injury data) to conduct analyses on program usage trends and effectiveness. Sensitive and highly sensitive personal information that is stored with Work-Fit (e.g. the notes that an Athletic Trainer documents when providing first aid treatment) would only be released with your written consent.
